PDC COORDINATORS

Your role in Project: Dentists Care is vital to insure the long term success of the program.

From recruiting volunteer dentists to collecting and submitting statistical data, this area of the FDA website is designed with your success in mind.

The PDC Volunteer Participation Form is provided to assist Coordinators to recruit volunteer dentists. This form explains the program and can be given to any qualifying Dental professional. Dentists who wish to participate will complete the form and return it to you, so that they can be added to the Volunteer Dentist Roster.

PDC Coordinators then report back to the FDA important statistical data from volunteer dentists and PDC Associate Members regarding the actual services provided. This data is reported quarterly, and is used to maintain the integrity of the program, as well as to identify improvement opportunities.

In order to help collect and process the information, please provide your individual volunteer Dentists with the forms below, so that they can submit the needed information back to you. Please review these forms so that you may better understand what is being asked of Volunteers.

1st Quarter (Jan-Mar) Volunteer Reporting Form
2nd Quarter (Apr-Jun) Volunteer Reporting Form
3rd Quarter (Jul-Sep) Volunteer Reporting Form
4th Quarter (Oct-Dec) Volunteer Reporting Form

In any given quarter, having collected all Volunteer Reporting Forms from all Volunteers, you will add the totals and submit your "master" quarterly report.
Coordinators may submit quarterly report forms via fax using the following forms below:

In any given quarter, having collected all Volunteer Reporting Forms from all Volunteers, you will add the totals and submit your "master" quarterly report. We have created a master quarterly reporting form that allows your data to be submitted online here. For purposes of consistency and uniformity, we recommend using the online version.

With your valid PCD username and password you may download and print the official PDC CE Certificates (the document contains two certificates, the first being for dentists and the second for hygienists). If you have misplaced or did not recieve a username and password, please contact us below.
