RELIEF FUND
The Florida Dental Health Foundation (FDHF) and the ADA Foundation offer a charitable Relief Fund to help Florida dentists and their dependents (including former dependents of deceased dentists) who suffer financial hardships because of misfortune, age, illness, physical or other disabling conditions or death.
Grants may be offered in a lump sum or in monthly installments, usually for six months or one year. Relief grants also can be renewed if approved.
Although the FDHF oversees the application process, membership in organized dentistry is not an eligibility requirement. All licensed Florida dentists and dependents of living or deceased Florida dentists are eligible to apply for this grant. The FDHF and ADA jointly deny or approve requests.
For more information about the Relief Fund or to request an application, contact Tom Norman, FDHF Director of Development at (800) 877-9922, Ext. 166 or email at tnorman@floridadental.org
